I don't know about the port issue - but calling it a visa on arrival is a little silly (although they do). There is no paperwork and no cost - it is just a passport stamp that they call a visa. I have crossed many, many times by land and air (although never by sea) - it is very simple.

Yes, and others (notably Egypt, Turkey, and Jordan, among others) it's more like a tourist or entry tax, as all you have to do is fork over money and they give you a visa.
