Sorry for you feeling insulted, but your question was a bit like "a friend of mine visited a very tall steel tower In Paris, but I am waiting for him to mail me the name". There are AFAIK several lava tubes you can visit in Iceland (also by yourself) BUT I know if only one vertical lavetube where tourists are routinely lowered into - and it has become very, very well known in the few years they have been operating the business. Just try to google and see how many links to newspaper articles etc you get.

I've used a very similar itinerary last year in october. We have been venturing to southern part of Iceland and had 10 days. That was quite a tight schedule with around 1500km of total driving. In the end it was definitely worth it but beware of a lot of driving :)

If you can I recommend driving all the way around the Snæfelssness peninsula, and as for "pretty fishing villages" you´d be hard pushed to find a prettier fishing village than Arnastapi. A must see.
I´d say your itinerary is good, although I´m not too sure you´d be able to fit all the adventure trips in, especialy as you are planing on going to Jökulsarlon.
April is not officially tourist season so bear in mind a lot of places outside of Reykjavik won´t be open. Be prepared to eat at gas stations.
You definitely won´t need a 4WD. For car hire I strongly recommend www.carsiceland.com - I have used them many times and always got fantastic customer service from them.
