It is worthwile to stop in Lviv on your way from Kraków to Kiev. There's a direct Kraków - Lviv sleeper train, the tickect costs about 150 zł.
Bus is a less comfortable, but slightly cheaper option, see: [http://www.wencel.com.pl/firmaGB.php]
The cheapest method is to cross the border on foot.
Kiev to Košice - the cheap and quite comfortable option would be to take an overnight train to Uzhgorod and change there for a bus to Košice. See www.uz.gov.ua for train ticket prices within Ukraine and use the Search function to find previous posts about the bus.
There's no direct transportation from Košice to Kraków. The simplest method would be train to Poprad, bus to Zakopane (strama.eu, seasonal) and another bus to Kraków - use the search function to find more details.

When are you planning to travel? On most days you don't need to book anything in advance.

I think we'll be travelling from Krakow to Kiev on the 18th, and to Kosice on the 22nd or 23rd.

You mean October, yes? The dates are not Friday and not Sunday, so no need to book much in advance. It might be wise to book sleepers a day before, but you should also be OK buying tickets right before departure.
