Don't visit Bangkok more than once on such a short trip, and avoid backtracking when possible. Flying into Phuket or Krabi, and home from Bangkok would be ideal. Transferring from Phuket to Krabi by boat via Phi Phi, or vice versa is straightforward. Then you could fly to Bangkok from the other airport, or get the train, as suggested, and avoid Bangkok. Instead of Kanchanaburi, you could look at Khao Sok NP, which is en route from Krabi or Phuket to the train, plane or bus to Bangkok from Surat thani.

Thanks for the support obviously OP would need to do the research for specific dates but with a number of airlines flying to both as long as they do not look for 2 single tickets and instead use the option of a round ticket flying into one and out of the other this should be a no brainer

The only issue is that sometimes these are harder to find on the simple computer searches but a good agent can help and certainly from the UK I have been found some excellent deals (into Phnom Penh and out of Kota Bharu for example) that were far cheaper than the simple return flights to KL
