Fly to Vietienne - 2 or 3 nights
Bus to Vangvienne - 2 nights
Bus to Luang Prabang - 5 nights
Chang Mai - 3 nights
Fly to Phuket 3 nights

Seems good to me. About right in terms of how much you can cover without rushing around.

Here's a Thailand Guide and a Guide to Laos that has more details for most of your route

Go straight to Luang Prabang, not much to see in Capital?

Or even better, go to Chaing Mai, then Border, then down Mekong to LPB, then fly LPB to BKK....

Don't forget the 2 day slow boat from the Thai - Laos border in the north to Luang Prabang!

imo Avoid Vang Vieng , Muang Ngoi a day from LP is a much more pleasant riverside place, on the backpacker trail and getting busier every year but without the same excesses of Vang Vieng( not yet anyway!) Lovely walks in the area.
