1) I'm pretty sure you need a visa prior to arrival in Myanmar. Thus Bangkok or somewhere else is what you need to do.
2) Not that I know of. You can certainly get flight from Bangkok and I also think from Chang Mai and/or Chang Rai.
3) Depends on your style and how you travel, maybe about $25 a day.
4) Very easy. You can go by train, bus, shared taxi or fly.
Have a great time in Myanmar, in my opinion it is safe to go now. I went last year and had a great time. Remember that the US dollar you use for larger payments need to be newer and VERY CLEAN. Also they should not start with CB at the beginning of the serial number.

1).. even better, get your visa in your home country or whichever Myanmar embassy is closer to you. Otherwise try Bangkok.. At least 3 working days at the moment.
2) no. Bangkok or Chiang Mai (one flight per week) are the options from Thailand. There were plans for flights from Siem Reap and Ho Chi Minh City or Hanoi.. but they have been postponed for the time being.
3) yes
4) lots of river boats. Few of the beach resorts hire out catamarans.
6) only a couple of islands open to foreigners in the Mergui archipelago. One is the Andaman Club resort.. pricey!
