Here you have the Laos-page of project visa, which gives you the links for all Lao embassies and visa-regulations. The usual scoop is that you get the Visa at the embassy - you either go there yourself with your passport, the money for paying the visa and photos (this might be a bit quicker and cheaper) or arrange the visa via your guesthouse for a sevice-fee. Like this, you give your passport, the visa-fee and photos to the clerk at the reception and he'll send a courier to the embassy where he'll pick it up when it's ready. With Laos in PP, I remember that it took two days in 2005, but I think there was a slightly more expensive express option where you receive the visa the next day...

You can NOT obtain a Laotian visa on arrival at the Cambodia / Laos border - it's an official crossing-point but not one of them with visa facilities!

You CAN get a Cambodian visa on arrival at the Voen Kham - Don Krolor land border. Cost is $20 + "service charge" of $1-2
You CANNOT get a Laos visa on arrival at the Voen Kham - Don Krolor land border. Get it in advance in Phnom Penh or elsewhere.

As #1 and #3:
There seems to be no controversy re the Cambodian visa on arrival. It's available.
There also seems to be no controversy re the Lao visa on arrival. It is not available.
