Route from KL to Trang Islands

I will be in Kuala Lumpur in late November and am planning to work my way north and island hop. I would like to see the Perhentian Islands but am curious about what the best route would be to get back into Thailand and work back West and end up on the Trang Islands.

November is not suitable for Perhentian (monsoon season). Most places will close.
