When processing the renewal there will be less of a rush than at an airport, so the officials will have a chance to look more closely at the entrance stamps and visas in it. What they see might prompt them to ask questions...

Well for starters, Malaysian passport holders don't even go through immigration officials. They simply go through the automated gates which means no one will even notice if there's a British stamp or not in the passport.
However, if your "friend" did not enter Malaysia using their British passport, do not check-in for the flight back to the UK using the British passport at any airport in Malaysia. Reason being is because all foreigners who enter Malaysia get a very distinctive sticker in their passports at immigration, and half of this sticker is then removed upon exit of Malaysia. If the agent at the check-in counter notices that you do not have this sticker, they MAY alert immigration officials because it means that you did not enter Malaysia using that passport.
But in any case, Malaysian passport holders don't need a visa to enter the UK, so why not just check-in for your flight with the Malaysian passport?

Wouldn't the malaysian immagration be suspicious if the date you exit malaysia and the date you enter malaysia is more than 6 months?
For example having british and malaysian passport
Then you exit malaysia and live in england for 1 year
Then you come back to malaysia
Thats 1 year apart
That would cause a problem wouldnt it?
