What is the recommended way of going about things if one steps of the train in Lhasa and feels more than ordinarily sick and all the symptoms are those of AMS? If you should happen to end up at the hospital, can one expect them to do the right thing if things start to look grim - such as putting one on a flight out to lower altitudes (like Kathmandu f.ex.?

You need to do the right thing by making sure you have adequate insurance coverage that include medical evacuation. No-one (especially a Chinese hospital or travel agent) will fly you anywhere unless your insurance company is covering the expense (or you pay in cash!)

Normal reaction to AMS symptoms is to stay adequately hydrated, rest and take diamox (if prescribed) but if severe symptoms present themselves then get your travel agent to send you to hospital where they will put you on oxygen and most likely a saline drip. Other drugs such as dexamethesone and Nifedipin may be administered in the hospital.

However one of the best ways to treat severe AMS symptoms is the use of a Gamov or P.A.C bag which the more high end companies sometimes have. A bit of googling will give you more info on these.

This being said the incidence of severe AMS on arrival in Lhasa by either train or plane is actually very very low. The average reaction for most folks is a slight headache, unsettled sleep and reduced physical ability (much like a hangover!)

They do have oxygen on the train, there is a line fed right to your bed. I used it on several occasions throughout the trip.
