Pakistan visa from Nepal

Can anybody tell me how easy or difficult it is to get visa to Pakistan from inside Nepal? I am usa citizen. If you know costs of visa as well, that would be helpful,too.

Hi. I got a visa for Pakistan the day I applied for it. The embassy is not far from Thamel (Katmandu) and it was the most hassle free visa I've ever applied for, it cost me $30 USD.

I'm Irish though, so that may have helped with my application. They asked me for a letter from my embassy, but when I told them that the nearest one was in Delhi or Bangkok, they let me off.
