Swat, NOC or not?

Sure,you need a NOC to visit Swat.you have apply for it through a tour operator.

Any travel agency will do that?
This also means, booking a tour there is a must, isn't it?

Many tour operators in Islamabad/Rawalpindi who organize this NOC to Swat,Travel Walji's,Nanga Parbart Adventure,Karakorum Explores,Saiyah Travel.They will definitely charge for the services but you don't have book the who trip with them.

Till now just one agency answered my apply. Nanga Parbat adventure
But, they want to sell a package(tour guide) and I suppose others will do the same.
Which office is concerned to provide the NOC?

The application has to be given to ministry of tourism KPK,this particular ministry move it to home department KPK then they issue the NOC.

The home and tribal affairs department has asked all the federal and provincial ministries to inform the department at least seven days before visit of any foreigner to a specified place in Khyber Pakhtunkhwa.

According to a statement issued here on Monday, the home department asked the ministries/departments to keep a gap of seven days prior to the actual visit dates of any sponsored foreigners so the process of NOCs could be completed well in time.

The prior intimation was essential so that well coordinated and foolproof security arrangements could be made and issued to the authorities concerned.

The security providers, said the statement, required at least five working days in advance before actual visit dates for the visiting foreigners in order to avoid any embarrassment.

Yes you need NOC too travel to Swat and Chirtal. You can get it within 7 working days from Home Department Peshawar if you are applying through Pakistan based tour company, you may get it without applying through tour company but it will take long time and there will be no guarantee of getting it, they can refuse your application after keeping it for weeks!
