Money in Pakistan

I agree that Citibank, Standard Chartered, Alfalah and Royal Bank of Scotland (there is at least one in Peshawar for some reason) are the best bet for foreign cards. Visa, Maestro, Mastercard. There are plenty of banks that do not accept foreign cards so if you have issue then ask for directions to one of the above banks.
Peshawar (there are banks near Dean Centre) , Lahore (Mall Road) and Islamabad (Try the Citibank or Standard Chartered at the end of the Blue Zone) are all good for withdrawing from foreign cards. I would always carry enough cash for the north but I believe that Gilgit has an ATM that takes some foreign cards.

Bringing cash might save one from bank commission, either direct or incorporated in the rate, as the spread is really thin here in the open market.

Yes, there are ATMs in Pakistan but do they accept non-Pakistani debit card is another issue!?

I was in Lahore April/May 2013 and I tried to use my US and UK issued "Visa" debit cards in 6 local banks' ATMs without success. On enquiring, why was this and where can I use my debit cards to withdraw cash I was told to go to Standard Chartered Bank which I did and lo behold I was able to withdraw cash with both the UK & the US debit cards without explicit ATM user fee - but my UK debit card slapped on 3% or so for foreign transaction fee!!!

So look for Standard Chartered Bank - I found many in Lahore & Rawalpindi/Islamabad.
