There are many tourist around Islamabad lahore, Karachi and other major tourist sites these days. Avoid the tribal areas and rest have no safety problem.

No destination in world is cent pour cent safe these days dear theworldjog, but one's to avoid the troubled areas and continue the journey.
North (Gilgit-Baltistan and Chitral ) are safe, people are friendly and hospitable, no worries.
Avoid Karachi, the troubled city these days due to tarhet killings and unrest.
Near Peshawar, the smugglers should be avoided, close to tribal areas, so please be aware of that.
In Lahore and other tombs in Punjab, the rush hours should be avoided due to threats of attacks.

Comparatively the mountainous regions re safer then south but the over all security situation in Pakistan has gone better now.
You 'd need paper work like letter of invitation (LOI) from Pakistan based tour operator for your visa.

Get your Pak visa in home country. Pakistan has stopped issuing visas in host countries. You may need a letter of invitation from a Pakistan based tour operator in order to get your Pak visa in home country.
Northern areas such as Chitral, Kalash valleys and Gilgit Baltistan are safe to visit where as in cities visit Lahore, Islamabad, Rawalpindi and Peshawar. Avoid visiting Tribal areas as things are not very promising there.

You'd need to apply in your own town, where you live and belong.

Only those European tourists don't need a LOI for the Pakistan visa who've already been to Pakistan in the recent years otherwise Germans, Swedish, Austrians and French etc. they all need a LOI from a Pakistan based tour outfitter and sometimes along with a hotel reservation in Pakistan .
