We've lived in Africa since 1991 (does that qualify me as a longtermer?) and none of us has ever had malaria.
Generally, we don't take anti-malarials unless going into high risk areas. Here in Nairobi (low risk area) we do a combination/variation of the following: spray the cupboards etc every once in a while, rub in a bit of DEET, use mosquito coils and mosquito nets if the things get pesky.
We always use anti malaria tabs though in high risk areas such as Lake Victoria, Lake Malawi, Kariba etc. Then we use Malarone as a prophylactic and carry Co-Arinate (Artesunate/Sulfamethsxypyrazine/pyrimethamine|) as a treatment. I used to carry Co-artemer but, according to my local clinic, Co-arinate seems to have the edge now, plus it's got a simple colour coded, 2 tablet, 3 day treatment schedule. Now this isn't an advert but it also comes in Jnr: 20-39kg and Baby: <12.5 kg so there is no guess work involved in dosing the kids (if needed). As I said, we haven't had to use it yet but at least the clinical specs look good.
When we safari , it is often with the elderly and very young so I am always on the lookout for possible solutions and so I will appreciate any further knowledge on this subject.
