2Yes you can absolutely withdraw USD in Peruvian ATMs. Most major transactions in Peru happen with dollars (rent, car purchases, etc). Some ATMs have limits to how much you can withdraw (somewhere around $200 or 300) depending on the specific bank it runs through. But the short answer is that you should not have to worry about having a hard time finding dollars.

5Sure you can, but beware. We just got home from Peru and found that several times people would not accept our us dollars because they were 'too old' or had a small tear. We tried exchanging them at banks but not able to. Thankfully we were heading home via the usa so spent our tattered dollars there.

6What #5 says is true. It can happen. It should not occur that banks through ATM's give out US$ notes which they do not accept themselves because of damage, etc. Banks, exchange offices and the general public are hesitant or unwilling to accept such notes because the Peruvian Central Bank takes this attitude. It is a good practice to only get US$ notes out of an ATM at a bank office which is open at that moment. Check the notes very carefully and if there is one you don't like go to one of the tellers in the bank, tell your story and get it exchanged for a better note.
Once I accompanied a traveller to a travel agency in the center of Lima. After making the desired arrangements the agency asked for payment in US$. We went to the nearest (BCP) bank office and got the US$. Went back to the travel agent who refused one of the notes. Went back to the bank, told our story and they happily exchanged for a "good" note. Went again to the travel agent, and yes, finally mission completed.
This also means that if you bring US$ from outside Peru, make sure that they are impeccable, preferably new.
