1Those 4-5 days are non-consecutive (two weekends), right?
I have only limited Hokkaido experience, from a business trip to Sapporo, but managed to see: Otaru (in the early morning before work), hike Asahidaka-Sounkyo Onsen (in a very full day from/to Sapporo and hiking very quickly) and visiting Noboribetsu (before departure flight) in the late afternoon.
All of these were done on public transport and all of them I'd recommend. Also had a look around Sapporo itself, which I did not find all that interesting apart from the food.

2June can be a great time to explore Hokkaido, though you might still find snowfields at elevation at places like Rishiridake (not enough to require crampons and ice axes; leastways, not the year I was there in early summer).
For outdoors, my picks would be:
1. Rishiri and Rebun (two islands off the north coast)
2. Asahidake and Kurodake (two mountains in Daisetsuzan National Park; LP's Hiking in Japan guidebook warns of snow 'til July)
3. Meakandake and Lake Kussharo (active volcano, and a lake)
The LP Hiking in Japan guide gives some useful and interesting information on all three areas (except Kussahroko), including details for public transit access.

3Take the bus to Biei or Furano to see the fields of lavender and other flowers.
Ride the train to Otaru and walk the streets (canal, sushi restaurant street, steam clock, glassware shops, woodcraft shops, glassblowing demo, etc.). Nice view of the ocean along the way. Maybe find your way to the aquarium.
Visit the Sapporo Beer Factory. Good lamb lunch.
Train to Yoichi to see the Nikka Whisky Factory (self tour).
Train/bus to Lake Toya or Lake Shikotsu. Nice spas and near Toya are two volcanos.
Train to Mt. Yotei to see/drink the mountain water.
Train to Hakodate and nearby visit the Onuma marshland. See walking courses in green. http://www.japan-guide.com/e/e5356.html
Visit the only castle on Hokkaido, at Matsumae.

5One other nature site that is accessible for a day-trip from Sapporo is the hike around Tarumae-zan in the Shikotsu-Toya National Park. (Google for some pics.....Tarumae-zan is a volcano with a fascinating lava dome in the crater, and hike goes around the crater rim, with great views of the surrounding countryside and into the crater to see the steaming lava dome.) You get there by bus from Sapporo, and then you can get a taxi to the start of the hike.
