Depends where you are going and what you are doing (i.e. trekking) in Nepal.
It certainly would be a good idea to some research yourself, get a guide book...

You didn't mention trekking or trekking permits...

40US$s for the visa, and a guide book of sorts, on arrival is best and sort out the rest as you go.

No, it doesn't quite work how you think.

You're advised to read the stickies at the top of the thread. Especially note this bit of advice:

It is highly recommended that new users check out the Indian Subcontinent FAQ thread before posting a new thread as you may find many of the answers you seek there

That's true but there are only a few posts about Nepal in the FAQ thread and I don;t think they would answer the OP's questions coherently. The info on the destination tab is more useful.

CIWEC has good health information but maybe not good enough to base a decison on - check with a doctor.

immi.gov.bp has all of the visa information but not presented very wll. The first decision to make is whether to get a visa in advance or on arrival..

Answers about permits depend on what you want to to do; perhaps read through the information on the destination tabs above which might help to focus your questions. A guidebook is also very useful and worth investing in - it will assist in planning the trip and be a source of further information when you are in Nepal.

Good luck.
