USA CUSTOMS TRAVEL FORM
I AM LOOKING FOR SUGGESTIONS ON THIS TOPIC.I traveled overseas in December 2012 to visit my Mom and my extended family
in south eastern Asia.
I returned to JFK early in January 2013. I had completed all the Immigration and Custom
formalities at JFK Airport. When I reached home I found that the Travel
form which was submitted to the official was still left in my Passport.I am not sure
what is that I need to do with it. Should I mail it back to them , if so to whom and where to ?
I appreciate any input in this matter.

What Passport do you hold. What type Visa did you use to visit the USA?The two tone blue Customs is not a issue, its just a formality, if you need to declare anything etc, but Immigration 1-94 form is another matter.

Where is home?If this was a customs form asking about things you were bringing home to the USA in your baggage then just bin it.
If it's an immigration form (I94, etc), then it will be collected when you leave the USA for home.

Are we reading his post right? He traveled overseas to Asia, then returned to JFK.My guess is he's a foreigner living in the US, and he's just talking about the I-94 departure record which is, and should be, left in his passport.
