You will have to go to the office at Bureau of Immigration Cebu District OfficeP. Burgos St., Tribunal, Mandaue City 6014 - dont wear short shorts
No need for a photo but take a photocopy of your passport page with your details and a copy of the page with your entry stamp and 3100 pesos. You may get it done straight away or you may have to come back later to collect you passport.

You may be able to do it at the airport on Mactan island but I have never done it or seen a place where it could be done.If you are staying in Cebu City, it is likely you will pass by the Bureau of Immigration heading to Cebu City from the airport. It is just to the right as you pass by the Mandaue City Hospital.

I think that yes it is possible to get an extension on arrival at the airport but that there is something not quite satuisfactory about it in that it only gets you up to 38 days in total or something rather than the 59 days you do get if you extend at a BoI office. Getting an extension at an immigration office is not difficult, at all.
