Yes, food is included and tent too. You'll have to pay extra for sleeping bag, entrance ticket to MP and train ticket back to Cuzco.
No recommendation as for trekking agent - just look around in Cuzco and choose one.

Most hostels have a tour/travel agent. They're rather decent. Moat of the 'alternative treks' to Machu Picchu cost about 200USD with most things included except sleeping bag, lunch/dinner on the last day, and breakfast the first morning.

OK then how much should I expect to pay as an extra to enter Machi Picchu?
