2Transylvania in Romania (there are numerous places such as Brasov, Sighisoara, Sibiu). Timsoara in Romania also (although it is not classed as Transylvania).
Lviv, Ukraine (which has an overnight train to and from Krakow)
Sarajevo & Mostar, Bosnia and Herzegovina
Croatia is also nice if you want to end by the sea!

3There are whole regions of Eastern Europe which are not well serviced by trains so you might want to rethink this. There is very limited train service in Albania, Bosnia, Croatia, Kosovo, Montenegro, and Macedonia, also Estonia, Latvia and Lithuania. Buses and minibuses are the way to go in these countries.
It would help to know your interests, budget, etc.

If you mean also Central Europe - Poland has many must see sights.
Gdansk old city and Malbork castle,
Torun (old city of gingerbreads, birthplace of astronomer Copernicus)
Poznan - just amazing old city
Lublin - nice old city in Eastern Poland and small Renaissance pearl - Zamosc.
