Peru and Bolivia

Hey guys,
I have 3 - 4 weeks planned in Peru but would really like to go to Bolivia as well! Can someone please tell me whether the below activities can be done in that time frame and the best order and any other suggestions???
Arequipa including sandboarding in Haucancina
Inca Trail (we have that already booked for 7th September)
Lake Titicaca
La Paz
Uyuni Salt flats
Amazon
If anyone has good suggestions for an itinerary and good order to fit it all in would be much appreciated! would also like to go to one of the surf beaches in Peru if there are any suggestions there also?
