Does this mean you will have a driver with you always? Otherwise, your schedule is incredibly tight and relies on perfect transport connections in every location. Which won't happen.

Agree with emmef 100%. Furthermore, where are you crossing from Tajikistan into Uzbekistan? Hope you are up to date on the borders that are open and closed.

Couple of things:
1/ Do you know how you are affected by altitude sickness? If you don't know, then consider doing things the other way around - the climb up into the Eastern Pamir is significant and Karakul is ~4000m.

2/ Your itinerary leaves no time for chilling out. You should plan for a good few days rest as the travel is both long and arduous.

3/ You need to think about whether you will have a tour operator to manage your journey or whether you are arranging things for yourself. If its the latter, then you should plan some contingency for occasions when you can't find a driver/car.

Where is the best places to sleep from Sary tash to Khorog?
was thinking of Karakul or Murgab, Then Alichur, Langar, IShakim and Khorog but would like to hear from peaple having done the trip.

Then i sit worth to spend one ful day i Iskahim or not?

I will have acar with me with driver and would like to know the driving time between teh following places

Better to sit one day in Khorog than Irshaskim.

Khorog is a pleasant little city,Irshaskim is just a little village.

Sary tash Karakul
Karakol- Murgab
Murgab Alichur
Alichar- LAngar
Langar- Irshaskim
Iskashim- Khorog

This was with car and driver and some side trips.

Raki man, would you mind sharing where you got the car and how much it cost? Also, how'd you do with the altitude in that direction?

I used http://www.pamirguides.com/ who were really good. Cost for Osh- Khorog- Chinese border circuit over 7-8 days about $2000. This covered all meals accommodation etc and travel in Toyota, not a Lada. His prices are mid-range.

The guy who runs it lives in Murghab, his driver lives near Osh. Because he is a local he is good on side trips- including to Shakhty Cave!!

Altitude was not a problem for us- we had spent a couple of night at Lake Song Kol, 3500 meters- before we flew from Bishkek to Osh- which is also a must-do with a window seat!

At worst, you can sit in Sary Tash- about 3000 meters- for a couple of (boring) nights before going up to Karakol.
