Cubana poses an obvious obstacle, i.e. using a US credit card to purchase a flight to Cuba. We checked Copa (i.e. thru Panama) and Taca (thru El Salvador), but the cheapest (even cheaper than Cubana albeit with a connection) turned out Mexicana (thru Mexico).
Unfortunately unless your credit card was issued in Mexico, the Mexicana people will tell you that you can only reserve your ticket online, but then must go to the airport and pay in cash. HOWEVER, what they do not tell you is that you can also go to the Mexicana office (zone 10 in Guatemala City) and use your US issued credit card. A double win!

1Nothing new in your post.
FYI, you can call Mexicana in Mexico to book your flight, and pay over the phone with an American express card.

5Just to clarify, my original post referred to using a VISA credit card, which is the most common card people have (far ahead of AMEX). Mexicana will not accept a VISA credit card that was not issued in Mexico for internet purchases.

6That's right, you have never been able to use a non-Mexican issued Visa or Mastercard over the internet with Mexicana when booking a flight to Cuba. You along with #2 above have obviously just discovered this, but it has always been that way.
