I met 3 foreign tourists, American, Australian and New Zealander. They told me they used an agent named Heian Tours & Travels Ltd in Nepal. http://www.heiantreks.com/ This 3 guys said they paid US$400 / person, including transportation from Kathmandu to Lhasa, accomodation along the route, breakfast (no lunch and dinner included), tour guide, China visa, travel permit. It seems to be one of the cheapest in Nepal. It took them about 6 days to come to Lhasa, they didn't go to the Everest Base Camp.
