tibet permit in kathmandu

Can anyone confirm the current situation on getting a permit to tibet in kathmandu? I understand that you have to be part of a group but has it been affected by the recent protests? Do you need a chinese visa as well as a tibet permit to enter? If so, can you apply for these together in kathmandu or should we get a chinese visa before we leave home? Advice much appreciated!
