16Of course the joke here is that none of us know what sort of travelling the OP is doing. If a high roller moving in upmarket tourist circles then $s rule and widely accepted. If on small budget $s far less so and Dong essential. Also we don't know the home countryt - if US then $s are free of exchange rates.
I sometimes wonder why the first replier doesn't go back and ask pertinent questions so the answers can be suited. Nonetheless, it does give us all a chance to air our preferred currency options and then argue the toss with each other.
If bank cards are out, what is recommended - a huge wad down your trousers? I use cash up to an acceptable level, but for 2 month plus trips I don't want to carry that much cash. Essentially everyone has different circumstances and hence different solutions.

17We use credit card whenever a business looks reputable and has been around for a long time. Ie hotels, train tickets, airfares, larger stores and some large restaurants.
We pay in Dong at all other businesses.
We have Royal Bank, Bank card and the charges are $1 to $2 for using the ATM by the Vietnamese bank, 2% charge from the Royal Bank and a very reasonable exchange rate. (today's rate is about $1 Cdn = 20,900Vnd). All said it totals about 4% which IMHO is very reasonable as you aren't required to carry large amounts of cash of any currency. We normally make withdrawls of around 2 million dong and that's lasts us several days and there are ATM's in all cities and most towns.

18USD is accepted everywhere in Vietnam. Or you can change USD into Vietnam dong to spend with small expenses

20No 18: US$ are not accepted everywhere in Vietnam. OK they are in many places, but if you are going off the beaten track you need Dong. I cycle tour and stop in many remote towns and villages, I can't imagine offering dollars for many of the small purchases I make. Much easier to have Dong to start with. This was what I meant about finding out what sort of travel and to where.
